The patient was 22 y.o. at the time, a student and a dependent of her dad (his insurance). She asked our office to bill her mom. The insurance paid ZERO-- all to deductible. Who is responsible for the bill?

Whoever signed the consent for treatment/promise to pay part of the new patient information form. If the patient came to your office for the first time as an adult, and signed the form herself, then she is responsible. If you spoke to the mother and she agreed to pay before treatment was rendered, then the mother is. If the father is the one who signed or agreed to pay, then he is. But, it sounds like the patient came to you on her own as an adult; therefore she is the one who is responsible. If she is a full time student and is being fully supported by her parents, then it is up to her to ask them to pay the bill, not you. Good luck with this. These types of cases are difficult to collect sometimes.
