How does dental insurance work?

I thought 2 cleanings/year were covered by my dental plan - but turns out it was only every 6 months, so I was not expecting to pay for the cleaning. When the cleaning is not covered by my dental plan, what does the dentist charge me - the a la carte price, or the "discount" rate they charge the dental plan provider?
