I received a form today for a dental insurance and I don't know what dental doctor write in because I don't know which one from Children's Choice takes that dental insurance. My insurance company and my insurance is Access Dental.

Did you go see a dentist office? If so, call them and ask what you need to put in. If not, call Access and ask them what to do with the form.
