How can I get my Dental Insurance to Pay consistantly?

I get my work done at a dental school where the faculty and dental students work on my teeth. I pay out of pocket and my insurance is suposed to reimburse me. 50% of the time it does 50% it sends me forms that want more information. I went and had two wisdom teeth pulled. The dental school gave me the same print out to give to my insurance company. The company paid for one tooth, but the other tooth they claimed they wanted the residents license and tax id number. The tooth they paid for had the schools license number, the deans license number and the schools tax id number. They accepted the information for one claim, but not the other. The school tells me the information they provided is sufficient and the dental student who is not paid does not have a tax id number. I explained this to the insurance company, but they insist on the infomation. What can I do to be reimbursed?

I have gone to the schools as well, and it is a pain getting things paid for. Definatly document all your work and see if the school will let try again to help you push the paperwork through.. i'm still fighting something from over a year ago.. so instead..
